Requirements

When applying for any residential plans, please provide the following requirements during the application process.

Important: Many applications take time to process and some even result to decline because of inaccuracy of the submitted requirements.

Scenario 1: If you are the applicant and owner of a single dwelling house, please submit the following requirements. If living in a condominium, please see the additional requirement in Scenario 3.

• Completely Filled Up application form
• Photocopy of your ID (preferred Government ID)
• Latest Proof of Billing (past two months only)

Important to note when submitting requirements
(a) The address on the Proof of billing must match the address on the application form
(b) Provide the latest, for example, if today is January 2018, then submit either November or December billing.
(c) The name of the ID must match the name of the applicant written on the application form


Scenario 2: If you are the applicant and living with your parents, or relatives or just renting in a single dwelling house, please submit the following requirements.

• Completely Filled Up application form
• Photocopy of your ID (preferred Government ID)
• Latest Proof of Billing (past two months only)
• Authorization letter from the owner that he/she is allowing you to apply for a connection.
• Photocopy of the ID of the owner of the house granting the authorization

Scenario 3: If you are the applicant and owner/living in a condominium unit, please submit the following requirements.

• Completely Filled Up application form
• Photocopy of your ID (preferred Government ID)
• Latest Proof of Billing (past two months only)
• Admin Certificate 

Scenario 4: If you are the applicant and renting in a condominium unit, please submit the following requirements.

• Completely Filled Up application form
• Photocopy of your ID (preferred Government ID)
• Latest Proof of Billing (past two months only)
• Lease of Contract
